About the Mentzer Index
The Mentzer Index is a simple screening index used when evaluating microcytic anemia, to help distinguish iron deficiency anemia from thalassemia trait.
It is a screening index and has limitations. Other causes of microcytosis should also be considered, and confirmatory testing (iron studies, hemoglobin analysis) is required for diagnosis.
Iron deficiency anemia vs. thalassemia trait
| Feature | Iron deficiency anemia | Thalassemia trait |
|---|---|---|
| MCV | Reduced | Reduced |
| RBC count | Often reduced | Often relatively preserved / increased |
| Ferritin | Often reduced | Usually normal |
| Confirmatory evaluation | Iron studies | Hemoglobin analysis / genetic testing as appropriate |
